Commonwealth bank one opposite the Melasti Spa in Kuta, just up from the Bali Garden Hotel.. It has a small 2 teller bank attached. We asked what the maximum out was, 3 million rupiah at a time, but can use as many times in one day as the limit you have on your bank here in Australia.
The machine will ask for your card and then ask Indonesian or English, for language. Then follow the prompts. You will have to key in the amount you want as it doesn't show the 3 million key. It seems to have a round about way of doing it but it does work out OK. The notes you get will be printed or on a sticker on the top of the atm, this one only gave out 50,000 rupiah notes. REMEMBER your card comes out last. If you leave your card anyone can take your money as your pin is already in the atm. There are people watching for this to happen so be aware as you would anywhere.
The fees charged are up to your bank here in Australia. The exchange rate we found was quite good.
We also used another generic teller at Carrefour and had no problems, this one again had the prompts different to the Commonwealth and the atm's here in Australia.
It's all about being vigialnt at what you are doing.
