We have been using the same driver for over 10 years now and we never discuss the price.
We know we are paying double the 'market' rate but who cares - we are helping him and his family. At least when we call to say "are you available" to pick us up at some ungodly hour of the night or our plane is delayed an hour, we know his smiling face will be there waiting for us.
In a couple of months he is taking us through East Java for a week and the price will be "up to us".
It is also great to see how, over time, you have had an impact on someones circumstances in life.
