I wanted a hotel with gym when booking my holiday too but found that if you were content to stay in an average hotel and around Kuta it worked out well just to join Hammerhead Gym (google it, they have website with costs). I think it only costs $60AUD for a month which is cheaper than paying the extra per night for a hotel with gym facilities. Just a thought if money is an issue and you don't mind going out to the gym.
