with regard to the Malaria preventatives, I dont bother with them any longer but I do make absolutely sure I am not bitten by mozzies, day or night. this is pretty essential as Dengue Fever is also rife in Bali and is not prevented by Malaria prophylaxis.
I would get the immunizations because you can catch polio and Hep anywhere and Typhoid is more common than people think.
Murni's is a lovely restaurant in Ubud, as is the Lotus one. Dont miss the Babi Guling at Ibu Oka opposite the Palace.
Seminyak is full of lovely restaurants, just go for a wander.
The Bali zoo is one day trip you can't miss.
