My answers.
1. The further out of Kuta towards Seminyak the trend is towards better quality and some fixed price airconditioned shops. Still plenty of stalls though.
2. No big malls out of Kuta.
3. Easy afternoon walk from Kuta to Seminyak. Just dont do it in the heat of the day. In fact at low tide you can almost walk to Canggu and beyond (if you are feeling energetic).
