Good Toilets

I'm sure I'm not the only one who'd like to know where they are!
I've researched a few 'bibles' for good toilet reviews, but some them are a fair way apart - especially if the need is urgent! I need to know of some more places with good toilets for when I'm out & about, please.
And if you happen to know some that should be avoided at all costs, that would be helpful too. So far the 'good ones' I have are:
