for a full day - from around 8.30am till 5pm (and that included petrol to Bedugal, Ubud and back to Legian) so I would think you could get someone to stay with you for the 4 hours for between Rp150,000-200,000. Makro is fun and would certainly fill in some time but if you only have four hours, I'd get the shopping done quickly and then go somewhere nice in Sanur for lunch before going to the ferry. I'm not sure how big these public boats are and how much shopping you can take with you to Lembongan. Would also be a good idea to get to the boat early to ensure your tickets.
Maybe you could use an airport taxi to get you to Makro, then a normal taxi for the rest of the time.
