You can buy sim cards all over Bali these days. Often the sellers have a board, like a money changers board, out front of their shops showing the price of the different carriers and cards.
The Fuji shop opposite Matahari in Kuta Square are helpful.
The seller will put the card into your phone for you and check that the credits have been activated. You can top up same way.
