I'm going to Bali next week and am currently 6.5 months so not quite as far along as your sister will be but I found and took out cover with QBE. Covermore only covered to 3 months and QBE ended up being a lot cheaper anyway. QBE charged $67 for single or $133 for a family for a 9 days holiday.
They will cover for "Pregnancy related complications" but not for the birth itself (in case your sister is premature). I couldn't find anyone who would provide more cover than this and I work in the insurance industry.
She will just need to complete a "Preliminary Medical Appraisal" form which just asks for her due daten to be entered next to the "Are you currently pregnant?" box and sign it to declare she has a "pre-existing illness" they will contact her in 24 if all okay.
They did mine without having to contact my obstetrician but given how far along she will be they may need the "Doctor's Declaration" completed.
