I've never actually been sick from eating lunch at the warungs, but personally I steer clear of the meat/fish as I don't really fancy eating chicken that has been sat out in the sun all day.
Just go for a warung that looks busy - I always go for one that is a bit further on from the Inna Kuta Beach that is slightly set back from the beach on a little paved area. There are a few stalls here that sell a variety of dishes - nasi goreng, mie goreng, nasi campur, sate, gado gado etc.
