Many people have had Bali belly (in varying levels of seriousness) and they have eaten western foods in 5 star + hotels! It can be caused by not washing hands after handling money, peeling fruit, etc, but can also be caused by change of diet, change of climate, etc.
We've had Balinese come to Melbourne and have had Aussie Ache (the same as Bali belly but from the South!). And try visiting Adelaide (sorry Adelaide people, but it is true!). :)
