1&2 I think you would be more interested in a one way transfer rather than the lunch and activities, all the larger companies will do, ie: Island Explorer Cruises, Bali Hai or Bounty prices range from USD15-20 depending on if you want a pick up from your hotel. You can also organise a one way back from the Island this way too.
3 My husband took the local boat once, it was bad luck that the one time he takes it the motor brokedown and the seas were high. (Takes a long time to get there on one motor) Normally though it is about 45min and a good ride. The outrigger style is perfect for conditions here.
4 Accommodation depends on your price range, there are a lot of losmens down on the beach in Jungut Batu Village but not that close to Snorkelling. You can snorkel off the beach I think at Mushroom Bay, haven't tried there yet. Coconuts Beach Resort is closest to the reefs where there is some good snorkelling. They have water activities free for resort guests only from 9-11am (before the day guest boats arrive) and free snorkel use whenever you want it. Bali Hai and Nusa Lembongan Resort also offer activities, don't know if you have to share with day guests though.
5. Luggage can be left at the Perama Office at the Island while you look for accommodation or you can book ahead somewhere and get picked up from the public/perama boat.
6. Sorry don't know prices for Kuta-Amed but I'm sure someone will fill you in.
