you must negotiate with driver prior to job and agree on rate (unless small job use blue taxis)they have metres even though our driver is our friend still pay upon completion of day. We also have our driver eat with us if out for a full day it is amazing what you can learn just relaxing and him. Heaps of english spoken but is fun just to learn a little there is a great website bahasa indonesian then just have fun with the locals amazing what you will pick up. We have been many years ago in february, just like Darwin still wet season but mainly afternoon heavy rain there clear blue skys and high humidity. As far as orphanages I would ask your driver especially one that have been recommended from this forum I am sure the locals would know who is more in need than any of us. Also as far as having ice in drinks we have never had any problems but we always buy heaps of bottled water from supermarket (which are plentiful) and even clean our teeth with it. We have been travelling to Bali over the past twenty years and eventhough when younger we were much more adventurous we have had no problems (touch wood) so go and have a great time try all the local dishes at Warungs.
