I bought a Mentari sim card while I was in Bali a couple of weeks ago. I bought it from a little stall just in the back entrance of Mataharia and on your right, but they are available at lots of different places in Kuta.
It cost me 50 000rp, and came with 30 000rp credit.
Rates for SMS is 350rp
Rates for calls range from 850rp/minute for a local call, to 3750rp/minute, depending on the distance of the call. (km)
